Notice from Dr Vanessa Horton

Dear Patients,

I wanted to let you know that after a great 20 years of working at Wake Green Surgery, I am now moving onto pastures new. A decision like this is never easy to make, however, after much deliberation I have decided to take up the opportunity of a new partnership with the University Medical Practice in Edgbaston, which starts on July 1st.

I have thoroughly enjoyed my time at Wake Green Surgery and feel blessed to have worked with such a supportive and talented team.
I am grateful to have had the opportunity to make some longstanding positive relationships with so many of you over the years and I thank you for all your support. I know I leave you in the very capable hands of my colleagues. Any patients who are registered under my name will automatically be reassigned to one of my colleagues, although you will still be able to choose which GP you see at the practice by speaking to the reception team when you call / visit or alternatively selecting your GP of choice when booking appointments online.

National Diabetes Audit

This GP practice is taking part in an important national project about diabetes care and treatment in the NHS. The project is called the National Diabetes Audit (NDA). To take part, your GP practice will share information about your diabetes care and treatment with the NDA. The type of information, and how it is shared, is controlled by law and enforced by strict rules of confidentiality and security.

The surgery has been serving the people of Moseley and surrounding areas for over 50 years.

We aim to provide top quality primary health care in a friendly family environment.

At Wake Green Surgery we strive to offer you patient centred care, enabling you to understand the options available for treatment and care. We want you to be involved in and influence the decisions which are made about your care. We will try to provide you with as much information as you need to explain risks/benefits about your treatment options and your right to make a decision. Individual assessments and treatment will be based on your personal needs, choices and preferences.

Care Quality Commission

The partners, management and staff of Wake Green Surgery are committed to delivering a high quality service to our patients. Our recent Care Quality Commission [CQC] inspection raised no concerns with the direct clinical care provided by the doctors, nurse practitioners, nurses and health care assistants, but we were very disappointed that the CQC found the practice inadequate in some areas and as a result we have been placed in ‘special measures’.

We take the issues raised by the CQC report very seriously, and the doctors have been working intensively with the practice management team since the day of the inspection in December 2016 to address these issues. We are committed to bring about the changes required not only for this inspection, but also for the long term. We will all work hard with the CQC, Birmingham Cross City Clinical Commissioning Group, NHS England and our patients to ensure that we improve.
