We are accepting new patient registrations, this can be done online or in person, please bring 2 forms of identification.
Wake Green Surgery is part of Our Health Partnership and is a member of Birmingham & Solihull Clinical Commissioning Group (CCG). Wake Green Surgery is a proud member of MOBY (Moseley, Billesley and Yardley Wood) Primary Care Network working with Greenridge Primary Care Centre, Poplar Road Surgery, Baldwins Lane Surgery and Yardley Wood Health Centre.

You can now download the registration forms below, complete and deliver to the practice in person.

GMS1 registration form (for everyone to complete)
Adult Registration Form V1
Child Registration Form (Under 18 years)

Once you have registered with the Practice you may then register for our online Patient Access services. Please refer to our online services page for more information. Patient Access lets you use the online services of your local practice. These may include arranging appointments, repeat medication, secure messages, accessing your medical record and updating personal details.

We will also need you to complete our identity verification process before we can send you your log in details. This is just to make sure no-one can get access to your medical information other than you. Once you have registered we will then be in touch to invite you to a Patient Health Review so you can find out more about the practice and what we offer.

Please be aware however that if you live outside our practice boundary there may be some services you can’t access (eg home visiting). Please see our practice area on the adjacent map.
